Amigos de Guadalupe fulfills its mission by focusing on four strategies: Expansive Outreach (Alcance Comunitario), Trusted Information & Referral (Amigos de Confianza), Ta Comunidad Fortalecida) and Case Management (Compañerismo). The Housing Case Manager will be mostly focused on comunidad fortalecida and compañerismo .

Founded on the principles of justice and advocacy, Amigos de Guadalupe Center for Justice and Empowerment partners with the Si Se Puede Collective and local agencies to make social services more accessible to the Mayfair Community; to strengthen individuals and the community through education; and to address social inequities by creating strong leaders through community organizing.


Overview of Position:

Reporting to the Housing Manager, the Case Manager will evaluate and determine homelessness vulnerability of families in Santa Clara County, receive intake paperwork and complete eligibility assessments in preparation for program enrollment. The Case Manager will be a liaison between client and the Housing Manager by evaluating, interpreting and determining the needs of clients using effective listening and independent judgment. The Case Manager will perform a variety of clerical operational functions in support of the housing program. Role requires commitment to daily timelines, accurate data entry and record keeping, attention to detail as well as skill to respond effectively and tactfully to inquiries from clients. The case manager actively models and communicates the mission and vision and supports a culture of empowerment, team building, and open communication.
 

Key Responsibilities

  • Determine eligibility through intake documentation
  • Complete assessments and/or application to the appropriate program 
  • Accurately record time spent on assigned tasks
  • Maintain client files and electronic records in HMIS 
  • Maintain compliance of Destination: Home grant requirements
  • Return calls and follow up with current clients on scheduled basis
  • Establishes and maintains effective communication with clients, team members and others as appropriate as well as consults with other staff regarding client referrals to appropriate housing resources and supportive services.

  • Develops a comprehensive client-driven case plan with both short-term and long-term goals identified
  • Completes a timeline and measures for each goal
  • Monitors progress towards goals in regularly scheduled sessions
  • Creates, evaluates and adjusts individual case plans as needed 
  • Empowers clients to become involved in their own planning and goal setting
  • Refers clients to appropriate resources to assist with meeting goals
  • Maintains client files to include conversations, warnings, progress towards goals and documentation of any incidents
  • Collects data necessary to meet funding requirements and statistical reports
  • Effective communication skills

Qualifications

  • Bilingual proficiency in Spanish & English is required
  • BA degree recommended
  • Understanding of, sensitivity to, and respect for the diverse academic, socio-economic, ethnic, religious, and cultural backgrounds, sexual orientation, gender identity, gender expression, disability and age.
  • Must pass a background check

  • Of social issues impacting East San Jose residents

Physical Demands

To perform essential functions for the role, you should be able to:
